What is One Health and Care?

One Health and Care is a secure shared care record used across the Black Country, West Birmingham, Shropshire, Telford and Wrekin, and Staffordshire and Stoke-on-Trent.

It helps health and care professionals involved in your care access important information from different NHS and social care services. This means they can make informed decisions and provide safer, faster and more joined-up care.

How is your information used?

Your information is used in two ways:

  • To support your care by giving authorised health and care professionals access to relevant information about you.
  • To help improve local services by using information that has been anonymised, which means personal details that could identify you have been removed.

For example, anonymised information may be used to understand how many people are living with conditions such as diabetes, helping local organisations plan future services and support.

We want to hear your views

We are asking people who use health and care services to tell us:

  • How much they know about One Health and Care.
  • How they feel about information being used to plan and improve local health services.
  • Whether information about these changes and the opt-out process is clear and easy to understand.
